The difficulty level effects how many hit points opponents have, and how much damage they can do. Hard is actually hard (boss type monsters in particular), not just more challenging than Normal.
As a survivor archer on hard, I had to use a scorpion for the skeleton boss that shoots fireballs when opening 1 of the 4 tombs in the catacombs (made sure I got the last hit in for the experience points). If I stopped long enough to shoot at him, he would get a shot at me; I could kill him in a hundred(?) hits, he could kill me in two. Playing a warrior on normal difficulty, this opponent was difficult, but I didn't have to resort to hit and run tactics or scorpion traps. The other 3 boss type monsters here I attacked immediately after opening their tomb; IIRC the one that shoots fireballs was the one I withdrew from to kill off the lesser skeletons first, then attacked him (I didn't stand still for a long time or run straight at him, but did not need to take any great effort to avoid his attacks).

Some re-release versions of the game may already be updated to the latest 1.0034A version, as shown in the bottom right corner of the main menu, so would not need to be patched. The DVD version is already updated, and the patch (designed for the CD version) should not be installed.
If you want to get a feel for the difference, or check more exactly, you could use
iZakaroN's SaveEditor (or from
Kiya's site, with description) to boost a new character and check out a strong opponent under different difficulty levels (from the start, it would probably be easiest to head to the graveyard in the northern part of Aleroth).
Know Creature would come in handy if you want to check details (it requires level 19 to max, or 2,280,000 experience points, which you can get with the save editor).
